55歲女性，因汽車翻覆，由119送入急診，左足嚴重壓砸傷併開放性傷口大於15公分以上，經緊急手術診斷為 left foot crushing injury with dorsal foot huge avulsional wound, 1st toe proximal phalanx fracture, 2nd toe proximal phalanx and middle phalanx fracture, 4th toe near complete amputation with bone loss, 5th toe proximal phalanx and distal phalanx fracture, and 3rd and 4th metatarsal fracture。
1. 手術費用: 第四及第五趾行開放性骨折復位骨釘內固定手術，申報64036C一主刀一副刀，並不是附帶手術，若是五個腳趾都骨折而且也需要行開放性骨折復位骨釘內固定手術，則可以申報64036C一主刀二副刀，請詳查，謝謝。
2. 患者術後傷口紅腫，skin flap marginal necrosis，需靜脈抗生素治療，住院11天，無過長住院。若是無開放性傷口，10天刪5天，合理，但患者有skin infection and skin necrosis之併發症，total 住院11天並無過長住院。Thanks!

68歲男性，右膝關節已經接受過人工膝關節手術，因為被鐵條由大腿下端外側插入，傷口7x3公分，手術中發現，內側膝關節大量血腫，此為open knee injury。
1. 患者接受過人工膝關節手術又有open knee injury，手術清創後需住院接受靜脈抗生素治療，另外，若有膝關節感染，則需進一步行清創手術，住院五天，實為合理天數，請詳查，Thanks。
2. 右下肢CD<10 cm，每天一次，共四天。請詳查，Thanks。

47歲男性，由員警送入急診，為路倒病人，有濃濃酒味。經診斷為1. Head injury with brain concussion 2. Right big toe to 3rd toe fracture 3. Multiple laceration wound over face and right foot 4. Suspected C-spine injury。
1. 患者深夜入院，喝醉酒，無法神經學檢查。隔天一早發現有C-spine injury，家屬決定轉彰化基督教醫院治療，後來在彰化基督教醫院有做C-spine之骨折復位手術。
2. 住院以及所有檢查都是合理治療，合理產生，全數刪除，不合常理。患者喝酒，無罪可罰，卻懲罰治療的醫院，以後路倒的患者誰敢救？請詳查，謝謝。
3. 當然，救人是我们的天職，不會袖手旁觀的。
